It is really easy with saddling to blow through thresholds without noticing, particularly with introverted horses. This is actually one of the most common things we see with horses coming in for training with both young horses that have either been backed or begun the process, and also with more experienced horses who haven't ever really fully given their consent and had time to understand the process.
